Discuss kneecap dislocation and recurring patellar instability with Dr. Mesut Sançmış in Antalya. Explore assessment, rehabilitation and surgical options.
A kneecap dislocation occurs when the patella moves completely out of the groove at the front of the thigh bone, usually towards the outside of the knee. A partial slip is called a subluxation.
The kneecap may return to position on its own, but the supporting tissues can still be injured. Damage may also affect the cartilage or detach a small fragment of bone and cartilage.
Patellar instability describes a tendency for the kneecap to move out of position. Treatment considers both the immediate injury and the reasons it happened.
When to Seek Urgent Care
A newly dislocated kneecap needs prompt medical assessment. Seek urgent care if the knee looks deformed, becomes rapidly swollen or cannot support your weight.
Do not try to push the kneecap back yourself. Keep the knee supported and avoid walking on the injured leg while waiting for help.
Medical review is still needed when the kneecap appears to slip back into place. This page concerns treatment planning and is not a substitute for emergency care.
Why Does the Kneecap Become Unstable?
A twisting movement, awkward landing or direct impact can force the kneecap out of its groove. Some people have anatomical features that make dislocation more likely, even with relatively little force.
These can include a shallow trochlear groove, a high-positioned kneecap, ligament looseness or alignment that pulls the patella outwards.
The medial patellofemoral ligament, or MPFL, helps restrain outward movement. It can stretch or tear during dislocation. If it heals inadequately, instability may persist alongside any underlying anatomical factors.
Assessing Patellar Dislocation
The consultation reviews how the injury occurred, whether this was the first episode and any previous treatment. Examination considers kneecap tracking, joint movement, strength and leg alignment.
X-rays assess bone position and possible fractures. MRI can identify ligament damage, cartilage injury and loose fragments within the joint. CT may be used when more detailed alignment measurements are needed.
Repeated dislocations, persistent apprehension and a first episode with significant cartilage damage raise different treatment questions. The assessment should establish which problem requires attention rather than recommend the same operation for everyone.
Treatment After a First Dislocation
When the kneecap remains displaced, a trained clinician may need to return it to position, a procedure called reduction. Pain relief is provided as appropriate.
Many first-time dislocations can then be managed without surgery. Treatment may include temporary support, crutches and a guided exercise programme.
Physiotherapy works on movement, thigh and hip strength and control of the leg during activity. The duration of bracing and any movement restrictions should be explained individually.
Surgery may be considered earlier when there is a displaced cartilage or bone fragment, substantial associated damage or a particularly high risk of another dislocation.
Surgery for Recurrent Kneecap Dislocation
MPFL Reconstruction
MPFL reconstruction creates a new ligament restraint using a tendon graft. It aims to limit excessive outward movement of the patella while allowing normal knee motion.
It may be considered for recurrent instability when the soft-tissue restraint needs rebuilding. The assessment must also identify bone alignment problems that ligament surgery alone would not address.
Tibial Tubercle Osteotomy
A tibial tubercle osteotomy repositions the bony attachment of the patellar tendon on the shin bone. This can change the direction of pull on the kneecap or address a high-riding patella in selected patients.
The repositioned bone is fixed while it heals. The procedure may be combined with MPFL reconstruction when both are necessary.
Other Procedures for Selected Cases
A severely abnormal trochlear groove may require specialist assessment for trochleoplasty, which reshapes the groove. Cartilage fragments may need fixation or removal depending on their size and condition.
These procedures address different findings. Their availability and suitability must be confirmed during consultation; they are not interchangeable parts of a standard package.
Recovery After Kneecap Dislocation Treatment
Rehabilitation Without Surgery
Recovery involves restoring movement and confidence as well as reducing swelling. Exercises progress towards the strength and control needed for stairs, walking and later sporting activity.
A kneecap that has returned to position is not necessarily ready for unrestricted exercise. Twisting and impact activities should resume only as advised.
Recovery After Stabilisation Surgery
A brace and crutches may be required, with restrictions on bending or weight-bearing. The instructions differ between an isolated ligament reconstruction and surgery that also involves bone correction.
Rehabilitation takes several months. Return to sport depends on functional recovery and the procedure performed, not simply the appearance of the scars or the absence of pain.
Risks and Long-Term Expectations
Possible complications include infection, blood clots, stiffness, persistent pain and recurrent instability. Graft or fixation problems can occur, and an osteotomy adds risks such as delayed bone healing or fracture.
Further treatment may be necessary if symptoms continue or the correction does not heal as expected.
Stabilisation aims to reduce further dislocations. It does not guarantee complete pain relief or reverse cartilage damage that has already occurred.
Kneecap Dislocation Treatment Cost in Turkey
The cost should reflect the treatment recommended after assessment. Physiotherapy, an isolated MPFL reconstruction and a combined operation involve different resources.
For surgery, clarify charges for the graft, fixation devices, hospital care, rehabilitation and follow-up. Ask whether additional cartilage treatment would change the estimate.
Arrange the early review schedule and practical support before travelling for planned treatment. Confirm how physiotherapy and progress checks will continue after you return home.
A new dislocation should be assessed locally without delay rather than left untreated while arranging an overseas consultation.

I fortunately came across Dr. Mesut while researching orthopedic surgeons in Antalya after suffering a severe heel fracture in the United States. From the beginning, I was uneasy with the treatment plan proposed by my U.S. surgeon. After more than a year of being told that it was simply a “bad break” that would take time to heal, it was finally discovered that I had also severely sprained my ankle at the time of the injury, resulting in partial ligament and tendon tears that had been missed entirely.
When these tears were eventually identified, the solution I was given was ankle fusion surgery, an option that would have permanently limited my mobility. This did not sit right with me.
While in Antalya for my spouse's dental care, I had the opportunity to consult with Dr. Mesut at MSM Klinik. His team quickly arranged comprehensive testing to fully understand what was happening with my foot. Dr. Mesut confirmed the ligament and tendon damage but was very clear that ankle fusion was not the appropriate solution for my case. Through my time there, it was clear to me that he treats a wide range of complex orthopedic conditions, not just ankle cases, which gave me even more confidence in his overall experience. From our first conversation, I felt confident in his expertise. He immediately recognized the complexity of my injury, especially given that my foot had healed incorrectly over the course of more than a year and a half.
After returning to the U.S. and doing further research, I made the decision to go back to Antalya and proceed with surgery under Dr. Mesut’s care. One of the major factors in my decision was his use of stem cell therapy, an option that was never even discussed by my doctors in the United States.
From start to finish, the level of professionalism and compassion at MSM Klinik was exceptional. Everyone in the office was attentive, organized, and supportive. Eli was especially wonderful, managing all logistics and providing clear translation throughout the entire process, which made everything seamless and stress free. During my stay, I also received IV vitamin and supportive therapies at the office designed to aid healing and speed recovery.
I am now just 2.5 weeks post surgery and already able to walk at approximately 50% weight bearing. In the U.S., I would have been non-weight bearing for at least two months before even beginning this stage. My recovery has been faster and smoother than I ever expected, and everything is healing remarkably well.

My wife had a knee injection at MSM Klinik in Antalya in Turkey last year. She had been suffering from severe knee pain for several years and the injection really helped her regain mobility. So, we returned to MSM Klinik this year, where she has now had a second injection, which we expect will help her sustain her ability to walk at least another six months free of pain. My wife has been referred to NHS orthopedic specialists in the UK and is waiting for their diagnosis also. Waiting times are very long still in the UK, so the assistance provided by MSM Klinik in Turkey in the meantime has been extremely helpful.

I visited Antalya in early May of 2025, while experiencing acute sciatica pain going down my left leg. I got in touch with MSM Klinik and had the exam scheduled promptly after talking over the phone with the staff. I had the X-rays and MRI done very promptly at Yasam Hastanesi Life Hospital. MSM Klinik staff, Mr. Mursel, took me to the hospital and was with me there while I was getting the x-rays and MRI done. Then he drove me back to the MSM Klinik where we reviewed the results with the Doctor. The Doctor appeared very knowledgeable and offered an effective treatment plan. However, it was beyond my budget. I continued to manage my back pain with NSAID.
I'm giving 5 starts for the service and promptness, but not for any medical procedure as no medical procedure was done.
The Doctor, his assistant Mr. Murcel, and the office staff were very kind, professional and attentive to my issue, which deserves a 5 star review.
The MRI, x-rays and the consultation were reasonably priced.

A further episode is possible, but the risk differs between patients. Anatomy, ligament injury, age and previous dislocations all contribute to the assessment.
Is Kneecap Pain Always a Sign of Instability?
No. Pain around the patella can have other causes. The history, examination and imaging help distinguish instability from different patellofemoral problems.
Is MPFL Reconstruction the Same as ACL Reconstruction?
No. MPFL reconstruction addresses a restraint controlling kneecap movement. ACL reconstruction replaces a ligament inside the knee that contributes to stability between the thigh bone and shin bone.
Can Children and Teenagers Have Treatment?
Yes, but remaining growth affects the choice of treatment. Some soft-tissue procedures can be adapted for younger patients, while certain bone operations are unsuitable with open growth plates.
Does Every Repeat Dislocation Require the Same Surgery?
No. Some patients need ligament reconstruction, while others also need correction of a bony factor. Treatment should address the cause of instability identified during assessment.
